Proof of Age (POA) cards are issued by VicRoads on behalf of the Business Licensing and Regulation Section of Consumer Affairs Victoria which has sole responsibility. An application for a Proof of Age card can be made at some VicRoads Customer Service Centres (where indicated).

Application Forms
You can get an application form by:

• calling VicRoads on 13 11 71 (Mon-Sat 8.30am - 5.00pm)
• visiting selected VicRoads Customer Service Centres
• attending a Consumer Affairs Office. Find your nearest here 
• participating Australia Post Offices
• selected pharmacies or chemists

Requirements for a Proof of Age card
To obtain a Proof of Age card you need to:

• complete, but not sign, an application form
• present one of the original proof of identity documents from Category A (see below)
• present one of the original proof of identity documents from Category B (see below)

and take the form and the proof of identity documents to an authorised referee. The application form provides a list of authorised persons in front of whom you must sign your name.

You will need to sign the applicant's declaration in the presence of the referee.

You can then:

• take the completed application form and proof of identity documents to a Photo Point. Some VicRoads Customer Service Centres can process a Proof of Age card
• have a photograph taken

Your Proof Of Age card will be mailed to you by Consumer Affairs Victoria.

Fees

• Proof of Age application fee - $10

There are no fee concessions available for Proof of Age cards
