Where a person's gender is inconsistent with that recorded on the primary evidence of identity document (eg. birth certificate etc.), you must provide the following:
- A letter and a medical report from your gender re-assignment clinic doctor, and
- An updated primary evidence of identity document from the registry of Births, Deaths and Marriages (BDM)

If you wish to change your name to that of another gender (eg. a male name to a female name or visa versa) the standard Change of Name rules apply.
