Applying for Pre-qualification

Application Forms

To apply for pre-qualification or to upgrade an existing pre-qualification, a firm is required to complete and submit a Part A and a Part B application form for the Group. Information provided must address the requirements of the Eligibility Criteria for each Level included in the application.

Part A Form is the same for all Groups and requires completion of the firm details, business information, financial information, insurance information and a declaration in regard to the Conditions of Pre-qualification. (A copy of the Conditions of Pre-qualification is also included with the Part A File in the above documents.)

Part B Form varies for each Group and requires the Group /Category /Level(s) for assessment, management system details, firm work experience and relevant personnel experience. Also attached with this form are the following documents:

  • A VicRoads declaration for management systems
  • Management system checklists (Quality, OH&S, Environmental and Industrial Relations)

 

Eligibility Criteria
Eligibility Criteria documents have been developed for each Group. The criteria are nominated for each level and these requirements should be consulted to ensure that the application form addresses all relevant criteria. Note: This file also lists the Categories and Levels for the Group.

Completed applications should be sent to VicRoads and addressed as shown on the Part A form.
