Licensed Vehicle Testers - forms and fees

Licenced Vehicle Tester application forms

The following forms must be completed and submitted in order to apply for a Licensed Vehicle Tester licence.

LVT Application Check List August 2009 [DOC, 103KB, 6pp] - this is an overview of the application process and should be read prior to completing and submitting any of the following forms.

LVT Licence Conditions V23 September 2009 [DOC, 114KB] - this form outlines the general conditions under which a licence is granted.

LVT National Police Record- the licence holder is required to complete this form from the Victoria Police website and send it to the police for a national name check.

LVT Authorise Employee [PDF, 58KB] - this form notifies VicRoads when you authorise employees to sign certificates or test vehicles. Please attach a copy of the persons trade qualifications, accreditation certificate or card and drivers licence.

LVT Tester Application Form [PDF, 10KB] - the applicant fills out this form once the check list is completed and the application is ready to be submitted.

Forms for existing Licenced Vehicle Testers

LVT Licence Conditions V23 September 2009 [DOC, 114KB] - this form outlines the general conditions under which a licence is granted.

LVT Authorise Employee [PDF, 58KB] - this form notifies VicRoads when you authorise employees to sign certificates or test vehicles. Please attach a copy of the persons trade qualifications, accreditation certificate or card and drivers licence.

CARs Clearance Form [WORD, 41KB] - This form is used to notify the roadworthy section that issues identified at audit have been rectified.

SRS Report Form [DOC, 48KB] - this form is to be completed by an authorised dealer of the vehicle manufacturer for the make of vehicle being tested. The form is to verify that the SRS system has been checked  by the dealer, using the factory endorsed procedures and test equipment. There are two types of inspections, one is for a standard operational test and can be used for crash damaged vehicles and the other is for vehicles that have been water immersed.

Vehicle Damage and Structural Repair Report [DOC, 42KB] - this form is completed by an Insurance Company recognised, smash damage repairer business, to declare that a vehicle has been inspected to confirm it has been correctly repaired and continues to comply with the vehicle manufacturer’s standards and guidelines.

Request to change licensed premises details [PDF, 73KB] - this form contains the information needed to process a change of address or add an additional premises to the licence.

Licensed Vehicle Tester Audit Checklist [WORD, 128KB] - this form is to be used for internal audits and is also to be completed as part of the licence renewal process.

Licensed Vehicle Tester fees

Effective from 1 July 2009.

Fees are revised 30 June each year.

These are the fees payable by an applicant for a Licensed Vehicle Tester licence.
