Change of name

If you have changed your name you must notify VicRoads within 14 days to update your driver licence and vehicle registration records.

Change your name at a VicRoads Customer Service Centre

Download and complete a change of personal details form [PDF 63 Kb] and visit a VicRoads Customer Service Centre in person to present original documentary evidence of your change of name.

Find your nearest VicRoads Customer Service Centre

Your existing licence and/or learner permit will need to be handed in at the VicRoads Customer Service Centre. A new photo and signature will be recorded and a new licence and/or learner permit card will be issued free of charge and mailed to you in approximately 7 to 10 days. If you are unable to hand in your existing licence and/or learner permit card, a replacement fee will be applicable.

Registry of Births, Deaths and Marriages

The Registry of Births, Deaths and Marriages (External link) is the government agency responsible for change of name in Victoria. If you have any questions regarding change of name requirements or any of the certificates mentioned below, please contact the Registry of Births Deaths and Marriages.

Australia Marriage certificate issued by a Registry of Births, Deaths & Marriages in Australia
Overseas and your evidence of identity Category A documents (e.g. passport or birth certificate) have not been amended If you were born in Victoria, you will need a change of name certificate issued by a Registry of Births, Deaths & Marriages in Australia.

If you were not born in Victoria, and can prove that you have been a resident of Victoria for 12 months, you will need a change of name certificate issued by a Registry of Births, Deaths & Marriages in Australia.

If you were not born in Victoria, and cannot prove that you have been a resident of Victoria for 12 months, you will need to have your evidence of identity document amended by the issuing authority.
Overseas and your evidence of identity Category A documents (e.g. passport or birth certificate) have been amended.
Overseas Marriage Certificate and Category A and B documents that shows new name provided a link between both names can be established. (An overseas marriage certificate must be in English or accompanied by an approved English translation.)
Reason for the change of name Circumstances Documentation required
Marriage You have previously dealt with VicRoads using both your old and new names and your document details have been recorded by VicRoads. No evidence required.
Marriage VicRoads does not have information on your previous name. A Birth Certificate and either your Marriage Certificate issued by a Registry of Births, Deaths & Marriages in Australia or your Divorce papers. Documentation must show a link between both names.
De facto/domestic partnership You changed to your current name by Change of Name Registration. A further change of name registration certificate issued by a Registry of Births, Deaths & Marriages in Australia.
De facto/domestic partnership You did not change to your current name by Change of Registration, i.e. you assumed your new name. A Birth Certificate and additional supporting documentation such as a passport that shows both names or Centrelink document that shows both names. Documentation must show a link between both names.
Reason for the change of name Circumstances Documentation required
Change of Name Registration (issued after November 1986 in Victoria) VicRoads does not have information on your previous name. A further Change of Name registration certificate issued by a Registry of Births, Deaths & Marriages in Australia.
Deed poll (issued prior to November 1986 in Victoria)   The original Deed Poll certificate or a Change of Name Registration certificate issued by a Registry of Births, Deaths & Marriages in Australia.

Titles and honorifics No documentation required. This information does not appear on your licence and/or learner permit card.
Adding Junior or Senior as a suffix Evidence of Identity Category B document that indicates the name is being used.
